Coach Checklist

• Complete a Youth Ministry & CYO Coach Volunteer Form
• Be compliant with the Child Protection Policy of the Diocese of Arlington
• Submit signed Code of Conduct
• Submit signed Volunteer Risk Awareness Agreement
• Collect players’ Code of Conduct
• Collect players’ Permission Slip and Medical
• Collect players’ CDC Concussion Form

Youth Coaches

We welcome any high school aged youth to assist as a youth coach. This is a great opportunity if you have a sibling on the team and your parents will be driving to practices and games already! Please email [email protected] if you are interested.
